Infinite refresh loop on index.php

New to ZoneMinder here! I decided to repurpose a unit I had available for a CCTV setup. I'm using Ubuntu 18.04.2, got LAMP setup. Followed the install instructions found here.

I'm able to get it to load, but it refreshes the page ever couple of seconds. The login box title says "ZM_WEB_TITLE Login" & banner only shows up as "ZM_HOME_CONTENT" which when opened gives me an Error 404 on "localhost/zm/ZM_HOME_URL", figure both of these should be images that are loaded.

Tried multiple browsers, flushed cache. Tried it externally via another unit on the network, same issue but then it gives me, "unable to connect to zm db.sqlstate[08004] [1040] too many connections" after a few looped refreshes.

Any help is much appreciated!

I hit this problem after recently upgrading to Kubuntu 19.04. Turns our the ZM database didn't update as part of the upgrade process. Running "sudo zmupdate.pl" address my particular issue.
