Apparently everything was configured properly except the Remote Host Path, which was figured out using the Presets link. I figured this out by following the instructions from "How to setup an axis 213 with pan tilt zoom" even though I do not have an Axis 213.

Black screen using remote IP camera
This is my first post and I believe I have followed all the appropriate steps before getting to this point. I really hope someone can help me.
