black image with scrolling horizontal lines - bt878 Houppauge card

I have two Hauppauge cards that show up in Linux as PCI vendor & model ids of 0070:13eb which according to the list of bttv card should be used as "card=10,10" but I have tried 10,10 98,98 77,77 82,82 for the card ID in the modprobe options for the bttv module. But no matter what card ids I try or what config settings I use all I get is a black background with scrolling horizontal lines, I have also tried various values for pll (0 , 1, 2) in the same module options. I have zoneminder 1.34 installed on Ubuntu 16.04

This card looks like this:

On the card is lettering that says "Hauppauge 649000-02 LF"

I have tried many different configuration settings but each time I get the same or nearly the same black image with scrolling horizontal white lines.
I spent about two hours trying various setting tonight but no progress towards seeing anything other than this blackness with horizontal scrolling lines.

I am open to any suggestion as to what I should try next.
